Interim Care Manager

Interim Care Manager (Dementia Care Leadership) | Mission-Driven Home Care | Chicago Hybrid Role

Location: hybrid: office (Andersonville, Chicago) + field time + work from home 

Timeframe: ~Mid-July through early January 2027 

We are seeking an Interim Care Manager with a passion for person-centered dementia care to join the Chicagoland division of Renewal Memory Partners. The Care Manager works closely with the Care Director, Care Coordinator, and our team of Care Partners to deliver person-centered homecare to clients throughout the Chicagoland area.  

Responsibilities: 

Client Portfolio 

  • Serve as the main point of contact for assigned client portfolio 

  • Complete assessment visits, supervisory visits, and reassessment visits with clients 

  • Write client plans of care for clients and update them annually and as needed 

  • Manage onboarding, discharge, and annual rate increases for assigned clients 

  • Collaborate with Care Director to ensure growth towards end of year goals  

Care Partner Training and Supervision 

  • Assist in interviewing, onboarding, and daily supervision and Care Partners 

  • Address complex care issues escalated from Care Coordinator 

  • Lead orientation  

  • Complete initial competency evaluations for Care Partners  

  • Complete annual performance reviews and competency evaluations for Care Partners 

Community 

  • Collaborate with client services team to organize social events and special trainings with Care Partners to foster community building 

  • Represent Renewal Memory Partners at networking events hosted by community partners 

Compliance 

  • Collaborate with Care Director to ensure compliance to state regulations in client care and documentation 

  • Participate in quarterly Quality Improvement Program meetings 

 

Qualifications 

  • Bachelor’s degree; master's degree in field of aging a plus  

  • Passion for dementia care; passion for serving LGBTQ older adults 

  • Outstanding client services skills – capable of working within a concierge environment and building relationships with sophisticated clientele with high expectations 

  • Knowledge of Teepa Snow’s Positive Approach to Care and other dementia best practices a plus 

  • Experience in homecare a plus 

 

Compensation 

  • Salary + end of year performance-based bonus
