Renewal Patient Advocates provides 24/7, on-site support to Clients during medical emergencies. We exist so that clients are treated with dignity and respect during times of crisis – from home, to hospital, and everywhere in between. Patient Advocates ensure that each client’s full medical history and advance directives are known and honored by every healthcare professional in their service.

Our interdisciplinary team of Certified Patient Advocates includes both registered nurses and master’s-level social workers. Together, we take a holistic approach to client well-being.

Services include:

· 24/7 nursing house calls to assess and triage client concerns;
· Escorts to/from hospitals, plus on-site support within hospitals;
· Liaison with authorized family members and loved ones throughout any emergency;
· Consolidated documentation of advance directives and medical history (including medical information from multiple
physician/hospital networks);
· Communication, coordination, and advocacy across the healthcare continuum, including emergency responders, hospital nursing
staff, and physicians

Many advocacy clients are in good health, but seek peace of mind that comes with access to round-the-clock professional support.
